A unique opportunity has arisen to work in a modern, well equipped, multidisciplinary environment for people of working age with complex care, rehabilitation and/or life limiting physical and neurological conditions.
This permanent role is within a private rehabilitation care home based in Nottinghamshire.
JOB SUMMARY
To work within a small therapy team to provide high quality patient focussed care through assessment, diagnosis, development and delivery of individual treatment programmes. To take the responsibility for own caseload, within a defined area of service or for a particular patient type. To provide assistance in the supervision, support and education of unqualified staff and students. To participate in departmental audit in support of evidence based practice.
KEY RESPONSIBILITIES.
*Assess and treat complex patients with neurological and respiratory problems.
*Oversee and co-ordinate the work of the Therapy team, consisting of 3 therapy assistants, 3 physiotherapists, 2 occupational therapists and an activities co-ordinator.
*Provide supervisions and appraisals to therapy team members
*To provide support to therapy team members and other staff where appropriate.
*Ensure all therapy funded patients receive their treatments as agreed by an initial assessment, support management in the initial assessment of complex patients as required.
*To feed back to management or facilitate feed back via other professionals when treatment programs need to increased or decreased as appropriate, in order for management to adjust funding accordingly.
*To provide reports to support funding applications and ongoing justification.
*Attend Multi disciplinary meetings or at an outside agency venue as required.
*To liaise with other professionals within and outside agencies in regard to the receipt of therapies.
*Ensure staff and outside personnel operate within legal parameters for moving and handling.
*Ensure moving and handling assessments are completed and documented within 12 hours of patient admission where possible.
*Provide training for Pathfinders staff in moving and handling, positioning and other physiotherapy related interventions.
*To support HR in the recruitment and interviewing of additional therapy team staff.
Communications
*To communicate effectively, verbally and through touch, with patients to maximise rehabilitation potential.
*Communication skills of persuasion, motivation, explanation and gaining informed consent will be used with a variety of patients. To assess capacity, gain valid consent and to work within a legal framework with patients who lack the capacity to consent to treatment. Barriers to effective communication will regularly be evident e.g. loss of hearing, altered perception, expressive and receptive dysphasia, visual and mental disabilities, depression, pain and fear.
*To use written information to reinforce understanding of the patient's condition and the involvement in the treatment process.
*To liaise with medical, nursing staff and other health care professionals regarding patient care.
*To liaise and where appropriate advise patients, carers and relatives regarding patient care.
*To liaise with members of the multidisciplinary team and attend MDT meetings.
